Investigation Summary
At approximately 2:00 p.m. on February 17, 1999, Employee #1 was moving a 7 ft by 5 ft by 6 in. precast concrete wall. He was making normal lateral corrections as he proceeded when the wall support strut failed. He tried to run clear, but the lanyard of his safety harness became caught by the falling wall. He was pulled back and the 3,300 lb wall fell on top of him. Coworkers removed the wall and contacted Emergency Services. Employee #1 died of crushing injuries.
