Investigation Summary
Employee #1 was hooking a crane grabber to a vertical steel I-beam to move it to another location. In the process, the crane grabber struck the I-beam, knocking it over. The 8,000 lb falling beam struck Employee #1 on the back of the head and neck. He was transported to the hospital, where he was pronounced dead. The I-beam was standing upright, unsupported and by itself.
