Employee killed in fall when elevated platform shifts

Employee #1 was working from a safety platform elevated by a forklift. He evidently felt the platform move and attempted to jump to safety. Instead, he fell approximately 20 ft and died as a result of head injuries. The platform, which was not secured to the forklift, came down shortly after Employee #1 and landed beside him.
