Investigation Summary
At approximately 11:00 a.m. on January 14, 1998, Employee #1, age 20, was part of a crew working at a demolition debris recycling and sorting line. This operation consisted of a clamshell loader that fed a vibrating, fine-screened removal platform which in turn emptied onto a conveyor sorting line with five manned stations on each side. The sorters filled metal containers with debris materials such as cinderblock, scrap metal, or wood; a John Deere 482C1 diesel forklift, serial # T0482CM817272 was used to continually remove, empty, and replace these containers. Employee #1 left his sorting station to take a break. He was returning when he apparently tripped and fell into the path of the left rear wheel of the forklift, which was backing away from the sorting line. The wheel ran over Employee #1's head, killing him.
