Investigation Summary

Investigation Nr: 200650349
Event: 06/28/1999
Employee killed when stacked stage sections overturn

On June 28, 1999, Employee #1 was supervising temporary workers, hired from the local stage employees union hall, in loading staging from a fair into a trailer. They normally stacked the 125 lb stage sections wall-to-wall, but in this case, after the first section was stacked, they placed the stage sections front-to-back. As they were loading the sections, Employee #1 and a stage hand held them in place. The stage sections became unstable and several of them tipped over, landing on Employee #1's head. He sustained crushing injuries and was killed.
