Event: 10/09/1996
EMPLOYEE INJURED WHEN PIPE BURST

On October 9, 1996, two employees were working on a pulper. The two were in the process of adjusting a butterfly valve to eliminate the flow of hot water into the pulper. The butterfly valve was replaced two weeks prior to the accident because of problems. Employee #1 was at the butterfly valve control and Employee #2 was directly adjacent the hot water feed pipe, butterfly valve and damper. Employee #2 was adjusting the damper to attempt to slowly close or assist the valve control so it would close slowly. As Employee #1 operated the butterfly control the valve closed instantaneously. At this point there was a build-up of pressure within the system and the piping above Employee #2 ruptured. Hot water fell onto Employee #2 scalding him. The water was approximately 180 degrees Fahrenheit. Employee #2 was hospitalized with burns.
