Investigation Summary
On December 12, 1996, Employee #1 was rolling 30 in. diameter pipe onto the upper rack in a concrete pipe coating operation. The pipes were lifted hydraulically from a lower rack to the upper one. For some reason, Employee #1 placed himself toward the inside of the pipes at the exact moment that another pipe was lifted to the upper rack. The pipe rolled into him, crushing his head between the two pipes. Employee #1 died on December 27, 1996.
