Employee killed when struck by pipe falling from forklift

Employee #1, age 19, was rolling a 32 in. by 40 ft pipe onto a rack for a pipe coating operation. A large, powered industrial truck carrying an identical piece of pipe pulled close to the rack with the pipe extended approximately 15 ft in the air. The truck operator was waiting for the rack to clean so that he could place the pipe on it when somehow the forks holding the pipe tilted forward, causing the pipe to fall. The pipe bounced on the rack and struck Employee #1 in the head. Emergency Services was called and began transporting him to a local hospital. When Emergency Services realized that Employee #1 was beyond medical help they returned to the accident site to await the coroner, who pronounced Employee #1 dead.
