Employee killed when run over by front end loader

At approximately 11:25 a.m. on June 10, 1997, Employee #1, age 20, was directing off-road traffic at a cement mixing batch plant. The plant consisted of two sand storage bins, a pit, and a hopper area. Employee #1 was collecting manifest tickets approximately 73 ft away from the sand and gravel hoppers. At the time of the accident, two Caterpillar 996D front end loaders were hauling sand and gravel from the pit areas and dumping the material in the hoppers that fed the batch plant. There was also a dump truck positioned directly beside the sand hopper and another located at the gravel pit area. Employee #1 had walked approximately 59 ft from where his car was parked toward one of the loaders. He was either planning to ask the driver for a manifest ticket, or to direct the loader toward a sand pit. The loader operator was backing his vehicle down the incline when Employee #1 walked in back of it. He was run over four times by the loader's left rear wheel, and was killed.
