Employee injured in fall when scaffold collapses

At approximately 12:00 noon on or about June 6, 2000, Employee #1 was on a 12 ft straight ladder that was situated on a Perry scaffold. The scaffold overturned, causing the ladder and Employee #1 to fall to the concrete floor. He suffered multiple fractures and lacerations, and was transported by air ambulance to the hospital.
