Employee killed when crushed in machinery

Employee #1 was vacuuming out a chute for a material changeover when the vacuum hose became caught on the tilt control lever, activating it. The unit tilted forward and caught her between the frame and the chute leading to the auger that moved the product to the bagging station. Employee #1 was crushed and killed.
