FALL FROM DEFECTIVE LADDER

A journeyman electrician was working from a 3.7-meter stepladder, installing wiring for new lighting at a plant under construction. The stepladder had a bent step and a skid pad missing from one leg of the ladder. The electrician was also working on or near three uninsulated, energized 277-volt conductors. The employee fell from the ladder and died of injuries sustained in the fall. It is possible that he received an electric shock, which knocked him from the ladder. However, the medical examiner found no burn marks on his body.
