Employee Is Killed in Fall from Forklift Platform

On February 12, 2007, an employee was elevated to a work height of 10 feet by means of a Yale, material handling fork truck. He reached the 10 foot height and proceeded to try to pull a heavy box from a storage shelf, and position it on a make-shift wooden platform which was on the forks of the lift. The employee fell from this elevated position, in excess of 10 feet to the concrete floor below. He suffered massive head injuries and was killed. Employee was not using personal fall protection equipment.
