Investigation Summary
Employee #1 was walking near a 50-ton tree spader with other employees. The employee, for unknown reasons, fell behind the group and came within 10 feet of the spader that was moving at 1 to 1.5 mph. He bent down to pick up his gloves and was run over by the wheels of the spader. This caused multiple trauma and killed him.
