Employee's fingertip amputated in mechanical press accident

Employee #1, a press operator, was putting a can in press #6 when it tilted. He was reaching inside the press to straighten the can the press came down and pinched off the tip of his finger. Causal factors in this accident were that (1) the wrist pull-back device was not adjusted for operation over the top of the can; and (2) Employee #1 should have been using both hands to place the can in the sleeve, not just one.
