Investigation Summary
Employee #1 was on the second floor of a two-story home under construction. He was hammering in a portion of the plywood flooring near a floor hole that would become a stair entrance. There were no guardrails or barriers around the hole, and Employee #1 backed into it. He fell 17 ft, striking his head. Employee #1 was killed.
