Investigation Summary
Employee # 1 was opening the overhead feed gate at the load-out pad to allow feed to be dumped into a delivery truck. He was standing on the 18 in. wide catwalk of the feed delivery truck when he lost his balance and fell 10 ft onto the concrete pad. His head struck against the concrete curb, and he was killed.
