Investigation Summary
Employee #1 was working on a highway paving crew, responsible for marking and taking core cuts to ensure pavement quality. He was in the left lane of I-440 N, marking the locations for the core cuts, when a dump truck entered the work zone approximately 3,500 ft north of him. The truck backed up approximately 3,000 ft before striking Employee #1, who was facing south. He was killed.
