Employee killed when caught by trackhoe bucket

Employee #1, a pipe layer, was entering a trench box in an approximately 17 ft deep excavation as the operator of an excavator trackhoe was attempting to remove about 2 ft of soil from inside the box. Employee #1's head became caught between the trackhoe bucket and the trench box spool. He was killed. Apparently, the trackhoe operator's attention had been diverted by a traffic problem, and when he returned to digging he was unaware that Employee #1 had entered the trench.
