Employee killed when excavation caves in

At approximately 4:00 p.m. on December 6, 1996, Employee #1 was in a 22 ft by 24 ft excavation that was between 11 ft 8 in. and 12 ft deep. He was using a jackhammer to break up concrete when the excavation walls caved in, completely burying him. Employee #1 was killed. The vertical walls had no sloping, benching, or shoring, nor was a trench box in use.
