Employee killed when caught in a molding machine

Employee #1 crawled under a Hirsch EPS molding machine, model HE 1200, to remove foam that was stuck to the molding plate. Her coworker walked away to retrieve additional boxes so she could finish packaging the foam parts. The coworker returned minutes later to find Employee #1 caught between the cold mold sections of the molding machine. She had been killed. There were no witnesses to the accident.
