Employee killed in fall onto concrete floor

Employee #1, a warehouse order picker at a large home furnishings establishment, was working alone, pulling the last order of the day. The store had closed and when he failed to bring the item to the delivery dock or to answer a page, a coworker was sent to get the order. He found Employee #1 lying unconscious on the concrete floor. He had apparently fallen 14 ft 3 in. from the platform of his Crown order picker and sustained massive head trauma. A subsequent autopsy attributed his death to multiple skull fractures with associated bilateral subarachnoid hemorrhage, hemorrhage in the third ventricles bilaterally, and basilar brain hemorrhage. When the coworker found Employee #1, he was wearing his body harness and had the hook of his retractable lanyard attached to the frame of his machine. Apparently, Employee #1 was not using the fall protection correctly and had slipped from the elevated work platform.
