Employee died of asphyxia when pinned by plywood sheet

At approximately 10:30 p.m. on October 25, 1996, Employee #1 was removing forms from pier #6 on Lake Shore Drive below I-55. He was trying to pull out a 4 ft by 5 ft sheet of plywood when the board became stuck underneath the rail of the aerial lift basket, pinning him in the corner. As Employee #1 tried to move the vehicle, he apparently forgot the position of the driving wheels. He pushed the lever in the wrong direction, causing the aerial lift to move forward instead of backward. The plywood became pinned against his chest, crushing it. Employee #1 died of asphyxia.
