LACERATION OF HAND FROM ENGINE RADIATOR COOLING FAN

EMPLOYEE #1, THE SHOP FOREMAN, WAS ENGAGED IN TUNING UP A MILLER GASOLINE POWERED ELECTRIC WELDER. HE HAD REMOVED THE FAN SHROUD TO MAKE ACCESS TO THE CARBURATOR LINKAGE ADJUSTMENTS MORE CONVENIENT. WHILE LOOSENING THE GOVERNOR ADJUSTMENT SCREW LOCK NUT, THE NUT BROKE LOOSE AND THE EMPLOYEE'S HAND INADVERTENTLY CONTACTED THE RADIATOR COOLING FAN BLADES. HIS INDEX AND MIDDLE FINGERS, AND THE AREA BETWEEN THEM, WAS CUT. HE HAD BEEN EMPLOYED AS SHOP FOREMAN FOR APPROXIMATELY SIX MONTHS AND HAD A TECHNICAL BACKGROUND.
