EMPLOYEE FALLS FROM AN UNGUARDED OPEN STAIRWAY- INJURES BACK

Employee #1 was air cleaning a conveyor frame. He climbed a stairway and stood at the unguarded top step holding an air hose. As the pulley was turning, the air hose was caught in the pulley spokes. He was thrown over the open edge of the stairway and landed on a concrete slab 12 feet 4 inches below. He hurt his back, but the injury did not require hospitalization or treatments. The conveyor was three feet in diameter. The unused fence guard was 6 feet high.
