At approximately 1:21 p.m. on August 18, 1988, Employee #1, a packer, was examining, packing, and re-packing returned industrial containers containing calcium chloride or other substances. While inspecting the fifth container, serial #6001, Employee #1 used the wrong explosion-proof light to check the container for moisture and other contaminants that would interfere with the newly entered product. Employee #1 lowered a light approved for Class 1, Division 1, Class 1 Groups C & D, and Class II Group G atmospheres into a Class 1, Division 1, Group A atmosphere (acetylene), causing an explosion.
