Investigation Summary
EMPLOYEE #1 REPORTED TO WORK FEELING ILL. AFTER AN HOUR OF WORK HE RETIRED TO A WEATHER SHED ON A MOORED DOCK WHILE OTHER EMPLOYEES WENT FOR A COFFEE BREAK. HE WAS LAST SEEN SITTING IN A CHAIR WITH HIS LIFE VEST OFF. APPROXIMATLEY TEN (10) MINUTES LATER THE OTHER DECK HAND OBSERVED EMPLOYEE #1'S HAT ON THE DECK. HE SEARCHED FOR EMPLOYEE #1 BUT WAS NOT ABLE TO LOCATE HIM. A THOROUGH SEARCH WAS THEN INITIATED. HE WAS FINALLY FOUND NEXT TO, AND UNDER, THE BARGE BEING LOADED. THERE WERE NO WITNESSES. THE CAUSE OF THE ACCIDENT IS NOT KNOWN.
