EMPLOYEE #1 WAS WORKING UNDER A HOIST BOOM WHICH WAS ATTACHED TO A FORKLIFT. THE CHAIN SUPPORTING THE FORKS BROKE. THE FALLING BOOM PINNED EMPLOYEE #1 AGAINST THE PLATFORM RAILING, KILLING HIM.
