FURNACE PANEL WALL FELL ON EMPLOYEE; SKULL FRACTURED

EMPLOYEE #1 WAS REMOVING A POURED REFACTORY FROM AROUND THE WATER BOX OF A 46 INCH STEEL MILL REHEAT FURNACE. THE REFRACTORY WAS POSITIONED ALONG THE ENTIRE BASE OF THE PANEL WALL. EMPLOYEE #1 WAS USING A JACKHAMMER TO CHIP AWAY THE REFRACTORY. WHEN EMPLOYEE #1 HAD CHIPPED OUT ABOUT TWO THIRDS OF THE REFRACTORY, THE FURNACE WALL FELL, STRIKING HIM. HE DIED OF A FRACTURED SKULL.
