EMPLOYEE KILLED WHEN STRUCK BY FALLING CONCRETE PIPE

A PRE-STRESSED CONCRETE PIPE WAS BEING LIFTED, IN A VERTICAL POSITION, BY A FORKLIFT. EMPLOYEE #1, WHO WANTED THE PIPE TO BE LAID HORIZONTALLY ON THE FLOOR, INSTRUCTED THE LIFT OPERATOR TO LOWER THE PIPE. WHEN THE PIPE HAD BEEN LOWERED ENOUGH SO THAT ITS EDGE CONTACTED THE FLOOR, THE WEIGHT AND BALANCING OF THE PIPE WAS TRANSFERRED FROM THE FORKLIFT TO EMPLOYEE #1. HE LOST CONTROL OF THE PIPE AND IT FELL, CRUSHING HIS ABDOMEN. HE WAS KILLED.
