EMPLOYEE #1 WAS KILLED WHEN HIS HEAD WAS COMPRESSED BETWEEN AN 8000 POUND COIL OF STEEL AND THE UNCOILING MANDREL OF THE SLITTER MACHINE. THE ACCIDENT HAPPENED AS THE COIL OF STEEL WAS BEING TRANSFERRED FROM THE COIL CAR TO THE UNCOILING MANDREL.
