Investigation Summary
At 9:15 a.m. on January 14, 2012, Employee #1 was training a new coworker on how to start and operate a Black Brothers gluing machine (s/n 181130). Employee #1 was a temporary worker and he worked at the company for five months. Employee #1 was training the new temporary coworker who was on his fifth day of employment at the job. Employee #1 turned the machine on and had the guard on the feed side open. While he was trying to pick wood flakes off the moving glue rolls, the gluing machine accidentally started, exposing the ingoing nip points and causing an unspecified amputation.
