EMPLOYEE'S FINGER CRUSHED IN PRESS POINT OF OPERATION

EMPLOYEE #1 WAS OPERATING A PRESS BRAKE THAT WAS NOT EQUIPPED WITH A POINT OF OPERATION GUARD. HE WAS INEXPERIENCED, ONLY HAVING OPERATED THE MACHINE 21 TIMES PREVIOUSLY. HIS FINGERS WERE IN THE DIE AREA WHEN HE ACCIDENTLY TRIPPED THE THE PRESS BRAKE. HIS FINGER WAS CAUGHT BETWEEN THE PUNCH AND THE DIE.
