Employee killed when caught in sand muller machine

Employee #1 was a sandmill system operator whose regular job was to clean the north and south lines with a sand muller machine during the second shift. Employee #1 entered the muller on the north line sometime around 11:40 p.m. He apparently did not lock out the equipment as required before he entered. Meanwhile, an electrician had been assigned to check the electrical circuit on the sand conveyor belt located above the muller. He went to the control panel located on the east side of the muller and saw that the power was on. The electrician started the muller and the large mulling wheels apparently crushed Employee #1, who went through the discharge chute to the basement area and was killed. Employee #1's lock was still attached to his pants on a belt loop.
