Employee injured in fall from scaffold

Employee #1 and coworkers were using a coupler scaffold. They placed a plank from the scaffold to the roof of the Kaster Building so that they could reach a two point suspension scaffold. Employee #1 fell either from the plank or a ladder. He was hospitalized with back injuries.
