Investigation Summary
Employee #1 climbed a ladder and was standing approximately 10 ft 6 in. above a concrete floor. For some reason, he fell from the ladder and struck his head on the concrete floor. He sustained skull fractures and died. Nobody witnessed the accident. It is believed that the employee may have had a heart attack or a brain aneurysm.
