Employee killed under stack of particle boards

At approximately 9:30 p.m. on February 1, 1990, Employee #1 was writing the identifying markings with chalk on the end of a stack of particle boards after placing himself into a 17 in. area between the end of the roll conveyor and the shuttle conveyor. Without any advance warning, the stack of particle boards automatically moved from the roll conveyor onto the shuttle conveyor, striking Employee #1 in the waist. He was pinned underneath the stack of particle boards measuring 9 ft high by 24 ft long and 50 pieces high of 3/4 in. each. The boards weighed approximately 10,000 lb. Employee #1 was killed.
