Investigation Summary

Investigation Nr: 170120034
Event: 02/22/1991
Employee killed when crushed by scissor lift

Employee #1 was changing out a motor on a fixed scissor lift used to move meat products from the processing area to the packaging area. According to the only witness, an employee of another company, Employee #1 had raised the lift approximately 3 to 4 ft from the packaging area floor, cut off the breaker furnishing power to the lift, and blocked the scissor legs with a 2 by 4. The wood was placed in a channel on the lift frame with one end against a wall and the other against a roller that controlled the up and down motion of the lift. Both men were removing the motor. The witness stated that they were on their knees at the front of the lift, leaning under the lift platform. He leaned out from under the lift when the platform fell on Employee #1, pinning him across the back and chest. The lift frame and platform weighed approximately 1,000 lb. Employee #1 died at the scene of massive chest trauma.
