Employee killed when struck by falling bundles of lumber

At approximately 3:15 p.m. on July 31, 1992, Employee #1 was caught up at stacker #1 and moved to a stack of lumber with three stacking blocks that he had picked up on the way. Employee #1 placed them on top of a second bundle of the outer row of bundles so that more bundles could be stacked on them. The second row of stacked lumber was five bundles high and not blocked properly, causing the row to lean outward. Just as Employee #1 was turning to leave the stack of lumber, the top two bundles of the five-bundle row dislodged and fell on him. Employee #1 was knocked to the blacktop with the bundles landing on his chest and legs. He suffered severe chest and head injuries and died in transit to the hospital.
