Investigation Summary
A truck driver was backing a White 4000 tractor truck into a maintenance bay to remove a refrigerator trailer after work on it had been completed. The driver saw Employee #1 in his mirror as he began to reverse. A truck maintenance employee connected the air brakes and dollied up the trailer after the truck driver had backed the tractor under it. The driver then began to pull out of the bay. When he looked in his mirror, he saw the inspection box lid on the floor. He stopped his truck, dismounted to look at lid, and found Employee #1 under the back duel tires of the tractor. The truck driver and the truck maintenance employee stated that they had not seen Employee #1 even close to back tires of tractor, nor had they seen or heard anything at the time of the accident.
