Employee killed when buried in ore bin

Employee #1, a crane operator, had just dumped a hopper load of ore smelts into a transfer car bin at an ore bridge operation. He had apparently left the operator's cab and was standing atop the pellets in the ore bin to clean off the windows of his cab when the transfer car operator dumped a load of pellets. Employee #1 died of suffocation.
