Investigation Summary
Employee #1 had been assigned task of putting wire guard over gutter. He ascended a roof by ladder on south end of the building moving north to gutter on east side of the roof. Two coworkers on scaffold at north end back of a church heard something hit the metal gutter, one asked Employee #1 if he was alright and he said "yes" out of sight from the two coworkers on scaffold. Shortly thereafter coworkers on scaffold heard Employee #1 saying, "who" or something similar, and saw him in mid air falling. Employee #1 fell approximately 28 ft from north-east side of the roof that had approximately 50 degree pitch, landing on front part of his body breaking both wrists, and incurring massive head and body injuries. Employee #1 died approximately 23 hours later on December 15, 1989.
