Accident Investigation Summary
Summary Nr: 14551063 Event: 02/11/1992Employee Killed In Backward Fall From Metal Decking
Employee #1 was welding metal decking to the joists adjacent to the decking edge when he fell backward and dropped 15 ft onto the concrete floor. He sustained head injuries and was killed. The decking was not equipped with a standard guardrail and no personal fall protection was being used.
