Employee killed in fall from roof

Employee #1 was part of a six-member crew that was installing new felt and shingles on the roof of an existing apartment building roof. The building had two separate roofs, with two-man teams on each roof and two employees on the ground. One team was installing felt on one roof while at the same time the other team, which included Employee #1, was marking the felt with chalk on the other roof. When Employee #1's coworker stopped to refill the chalk container, Employee #1 removed his safety harness. He fell from the gable end of the roof and was killed.
