EMPLOYEE DIED IN FALL FROM AERIAL LIFT

A power line worker was working from an aerial lift 15.2 meters above the ground. He was not wearing personal fall arrest equipment or work positioning equipment. As he was tying off a new overhead power line conductor to a utility pole, he fell out of the aerial lift bucket. He died of injuries sustained in the fall.
