Employee killed, another injured by falling boat ramp

At approximately 11:00 a.m. on November 29, 1990, Employees #1 and #2 were installing hinge bolts on a boat ramp assembly. The boat ramp, which weighed approximately 4,000 lb, was suspended in place by a mobile hydraulic crane. The ramp was suspended from the crane hook with a wire rope sling that was connected to two staples on the ramp, and with a chain fall sling that was connected to one point on the ramp. When the suspension staple on the ramp failed, causing the ramp to fall, Employees #1 and #2 ran under the falling ramp. Employee #1 sustained head injuries and died on December 10, 1990. Employee #2 received head injuries that did not require hospitalization.
