Investigation Summary
At 12:00 noon on November 12, 1990, Employee #1 and a coworker were fixing a ventilation fan on the second floor roof of a building at a motel. Both employees left the roof for lunch. At 12:15 p.m., Employee #1 started back up the 32 foot portable metal ladder that was being used to access the roof. The ladder, which was not tied off, slid backward and the top of the ladder fell to the second floor railings. The employee fell, striking his head against a parked truck next to the ladder. He was taken to a local hospital, where he died two days later from head injuries.
