Employee's finger amputated by machine blade

Employee #1 was hosing vegetable particles out of a vegetable processing machine called an Urschel size reducer. The machine is left running while it is being cleaned and the guards are flipped up, out of place. When Employee #1 reached into the machine to wipe some particles off by hand, his fingers became caught between the rotating blade and a straight cut blade. The tips of the third and fourth fingers on his right hand were amputated. The employer had no lockout/tagout program.
