Employee dies after fall from lashing platform on ship

Employee #1, while standing on a lashing platform, was trying to install a 3-high lashing rod to secure containers on the deck of the vessel "Neptune Jade." Employee #1 lost control of the rod, which caused him to fall off the lashing platform. He died. The lashing platform had a guardrail around it, but the guardrail was only 2 ft high.
