Investigation Summary

Investigation Nr: 14333389
Event: 11/07/1991
No employees injured when pipes fall 31 floors to ground

On November 7, 1991, a load of sprinkler pipe was being hoisted to the 31st floor of the C & S Bank Plaza building in Tampa, Florida. The loose individual pieces of sprinkler pipe were loaded into a prefabricated open-ended steel basket, which was hoisted by two 20-ft nylon slings, which were rigged in single choker hitches. While employees were attempting to pull the load to the 31st floor, the basket tilted and the pipes slid out the back of the open-ended basket. Approximately 90 percent of the suspended load fell 31 stories to the ground, causing minimal property damage. No employees at the job site were injured.
